Program Facilitator – Elect-to-Work (Relief)


POSITION TITLE: Program Facilitator – Elect-to-Work (Relief)
SALARY: $16/hr
POSITION STATUS: Elect-to-Work (Relief/Casual)
HOURS OF WORK: Varied up to 40 hrs. /week

To provide direct services to adults with developmental and/or physical disabilities in a day program setting.

Direct Service Delivery

  •  Adhere to all Harmony Place Support Services policies and procedures
  •  Assist with delivery of programming in Harmony Place Support Services core streams: Social / Education, Leisure / Recreation, Health & Wellness, Life Skills Enhancement, and Community Outings
  • Assist with personal care and daily tasks (i.e. toileting, lifting, and assistance with meals)
  • Identify and organize activities in accordance with individual needs and the program focus
  • Provide input for Individual Support Plan (ISP) meetings as required
  • Guide individual’s to meet their ISP goals and objectives
  • Responsible for written documentation including communication notes, and incident reports, as required in accordance with agency policies
  • Perform general cleaning and maintenance of program area as necessary, reporting any hazards to the Health and Safety Representative or supervisor
  • Liaise with parents, relatives, other agencies and professionals as required
  • Participate in team meetings for the purpose of information sharing, program development, problem solving and brain-storming
  • Administer First Aid, CPR as necessary
  • PSW or equivalent diploma required
  • Previous experience considered an asset
  • Self-motivated, mature individual
  • Excellent written and verbal communication
  • Proven ability to understand the need of individuals with development/physical disabilities
  • Ability to maintain positive and professional communication with families, employees, other agency employees, and the public, while representing HPSS
  • Valid Vulnerable Sector Police Check or willingness to obtain
  • Certified in First Aid, CPR, and Safe Management or equivalent
  • Valid G License, personal vehicle and appropriate insurance considered an asset
  • Individuals must be able to carry out the physical demands of this position, including the ability to lift up to 40lbs, and to perform lifts and transfers
  • Ability to swim considered an asset
  • Bilingualism (English and French) considered and asset
